Physical Therapist
Position Overview
Neuro Rehab Collaborative is seeking a compassionate and experienced Physical Therapist (PT) with a strong background in neurologic rehabilitation to join our dynamic, multidisciplinary team. This role offers the opportunity to provide personalized, client-centered care in both clinic and home-based settings.
Key Responsibilities
Conduct comprehensive evaluations and establish individualized, measurable short- and long-term goals.
Develop and implement appropriate plans of care based on standardized assessments and clinical reasoning.
Deliver ongoing therapeutic interventions in alignment with each client's goals and needs.
Accurately document treatment sessions, client progress, and relevant communications.
Collaborate effectively with other members of the therapy and medical teams.
Educate clients and caregivers on home programs and strategies to support functional independence.
Maintain a clean, safe, and organized treatment environment.
Qualifications
Graduate of an accredited Physical Therapy program.
Licensed Physical Therapist in the Commonwealth of Virginia.
Board certification (e.g., ABPTS) required.
Clinical experience with neurologic conditions such as stroke, brain injury, Parkinson’s disease, or multiple sclerosis.
Experience working with older adult populations.
Familiarity with Medicare and other payer documentation and compliance requirements.
Current CPR certification.
Excellent organizational, communication, and time-management skills.
Ability to work independently and adapt to flexible scheduling.
